The NASA Engineering and Safety Center (NESC), created in the aftermath of the Space Shuttle Columbia accident to serve as an independent technical resource for NASA managers and employees, has reported on its initial assessments and lessons learned. Initial assessments were related to: the CALIPSO earth science satellite, the hypersonic X-43A research vehicle, the Space Shuttle rudder/speed brake, and the Mars Exploration Rovers mission.
